  • If I have never taken a GLP-1 medication which one should I start with?
As a general rule, we recommend our patients to start their weight loss journey with our sublingual semaglutide to reduce the risk of potential side effects. Starting with sublingual semaglutide before progressing to injectable tirzepatide can offer several benefits, particularly for individuals new to GLP-1 receptor agonists or dual incretin therapies like tirzepatide. Below are some reasons why this stepwise approach might be advantageous: 1. Patient Tolerance and Gradual Adaptation Lower Side Effect Profile: Sublingual semaglutide provides a gentler introduction to GLP-1 receptor agonists. This can help patients gradually acclimate to common side effects such as nausea, vomiting, or gastrointestinal discomfort. Dose Adjustment: Sublingual formulations allow for incremental dosing, which may minimize adverse effects and improve tolerance before transitioning to a more potent injectable therapy. 2. Convenience and Compliance Ease of Use: Sublingual administration is non-invasive and easier for patients who may be hesitant about injectables. It can build confidence and adherence to the treatment regimen. Accessibility: Starting with an oral or sublingual option provides a convenient, less intimidating entry point for therapy, particularly for patients new to incretin-based medications. 3. Evaluation of Efficacy and Suitability Trial Phase: Sublingual semaglutide allows healthcare providers to evaluate how well the patient responds to GLP-1 receptor agonists in terms of glycemic control, weight loss, and tolerability. Individualized Treatment Planning: If sublingual semaglutide proves effective, the transition to injectable tirzepatide may be unnecessary. Conversely, poor response can help guide dosing or alternative treatments. 4. Dual Mechanism Transition Preparation for Tirzepatide: Tirzepatide combines GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) receptor agonism, which may lead to more pronounced effects but also a higher side effect burden. Starting with sublingual semaglutide provides a foundation to tolerate GLP-1 agonism before layering additional mechanisms. Reduced Risk of Overstimulation: Patients are less likely to experience significant adverse effects when the introduction to GLP-1 therapy is staggered. 5. Weight Loss and Metabolic Benefits Incremental Weight Loss: Sublingual semaglutide can initiate weight loss and glycemic improvements, which may enhance the effects of tirzepatide when introduced. Metabolic Preparation: Early improvements in glucose and insulin sensitivity can make the transition to tirzepatide smoother and more effective.
